If you do not upload all of the following, you will not get paid!

4. IOI ‐ Direct Deposit Form
5. Local Earned Income Tax form from www.newpa.com
6. City of Pittsburgh Local Service Tax Exemption Certificate 
7. I‐9 signed and submitted Need Photo Copies of ID Proof 
8. Adult Application for Scouting America  
9. Pennsylvania Child Abuse History Clearance
10. Pennsylvania State Police Criminal Record Check 
11. Federal Bureau of Investigation Criminal Background Check 
12. Youth Protection Training Yearly Update 
13. Unlawful Harassment Prevention 
14. Hazardous Weather Training
15. Scouting America MedicalForm  - Bring this with you to camp

Under 18 also needs:

1. Work Permit ‐ Out of State (non‐PA) needs a local work permit
2. Under 18 Permissions ‐ Signed by parents
